Hi John, first of all, my sympathies.

Your main questions:
1) They will decide based on whether they think plaster alone will stabilise the breaks. Sometimes they give you a say. There' a risk of infection with surgery, and interference with the knee if they put an intramedullary pin in (a rod down the larger of the two lower leg bones - the tibia), and some people don't want to have scars from an operation, or really don't want an anaesthetic or whatever.
2) Yes, likely to be in plaster after any operation - but probablyy only below-the-knee and for a shorter time than if you don't have surgery.
3) If it's plaster only it will be as big as the one you have now, as it's important to immobilise the joint below and the joint above the break (ankle and knee respectively) to make sure the bones stay where they should be.

Keep your leg elevated (e.g. up on pillows or a large bean bag) as much of the time as possible to allow blood to flow back to your heart. This helps with the pain and swelling. The pain does get better with time; the first 2-3 week are worst.

Re showering: you can get a cast-protector which is basically a very large plastic bag that totally covers the cast. however getting into and out the bath/shower can also be a problem. You may have to accept washing with a facecloth for the moment. I'll try to post more about coping and showering and stuff tomorrow.

ORIF plate 18 June. Cast problems (allodynia) so boot from 25th but NWB.
25 July start PWB in boot, 7 Aug PWB without boot. 14th Aug few steps FWB.
9 Sept 10-steps jog 25 Sept: PT says go run!
